Welcome to the North Simcoe Muskoka Self Management Programs.
Self Management programs are helping people throughout the region. Programs are designed to inspire excellent self care by the person who lives with chronic conditions.
Chronic conditions challenge everyday life. Examples include heart disease, stroke, chronic kidney disease, respiratory disease, diabetes, cancer, HIV and chronic pain. How people manage their symptoms, medications, eating and physical activity has a powerful impact on their well-being and health outcomes.
Programs include workshops for health care providers to excel in their work with people who have chronic health conditions as well as workshops for people themselves.
North Simcoe Muskoka Self Management Programs are funded by the Ontario Ministry of Health & Long-Term Care through the
Ontario Diabetes Strategy. This provincial wide program supports persons living with chronic conditions in every Local Health Integration Network region in Ontario. In North Simcoe Muskoka, the program is housed by the
South Georgian Bay Community Health Centres in Wasaga Beach.
